We use this contact form app to collect our customer requests. This is absolutely helpful and the form itself is neat and good-looking. Plus, the form is able to integrate with our email service that could be used to conduct the promotion. And their tech support is fantastic, there are some small features that need to be adjusted after the plug-in, representative reply to our email within few hours and fix the issue ASAP. Excellent contact form with great customization. We needed a form that would notify various email addresses based on the choice made by the user. There was a minor glitch upon initial install, but Duke was responsive and quickly helped with the technical issues. Price is great for the features it provides. Highly recommend!

Very patient & very fast with great focussed follow through over several things that I needed help with on my first 2 custom forms. This is a very neat form builder, I'm glad I chose it, more features than a lot of the others I looked at. I need to get measurements from my customers and product use case so not being limited to 6 input fields like a lot of the apps I looked at was one of the winning points for me. Very generous with features & integrations. They let me know there was some jquery issue and overlapping elements with another app I use all the time and fixed everything really quickly.
