I'm sure this app is wonderful. It certainly looks great! but it's difficult to see from the app details to tell you that extra fields in the checkout aren't available to stores that aren't on the Shopify Plus Plan. There's a little note "For ShopifyPlus Merchants" in the images above in case you missed it. I hope this review is published, as it will save loads of people from wasting their time.
